When Jerry Solomon was admitted to Banner Del E Webb Medical Center for some esophageal tests, it was discovered that he was having heart complications, and that emergency heart bypass surgery was required.
During the discharge procedures, Jerry really tried to pay attention to the instructions given for after surgery care, but all he really wanted to do was get back home.
A Sun Health Care Transitions nurse went to Jerry’s home, and helped the family to set proper healing expectations, went over medications, and provided an overall piece of mind about his recovery.
