Jennifer Drago attended the Case in Point Platinum Awards ceremony today in Washington, DC., where our Care Transitions Program was recognized as the 2013 Winner in the Transitions of Care Program category!
The Case In Point Platinum Awards is a unique awards program that recognizes the most successful and innovative case management programs working to improve health care across the care continuum.
This fourth annual awards competition sets the standard for programs that deliver sustained success across a variety of disciplines and settings in the overarching continuum of care coordination.
“When I went to the podium to accept the award, I accepted it on behalf of our team and I mentioned how fitting it was to receive this during Nurses Week because the award pays tribute to the truly amazing RNs and LPNs that we have working with our community members each day,” she said.
