Sun Health was proud to be recently recognized in the Surprise Independent for its Care Transitions program – a community health program that supports patients as they transition home following a hospital stay and helps them to self-manage their chronic health conditions.
Sun Health Care Transitions is honored to be one of 46 sites participating in a Centers for Medicare and Medicaid Services demonstration project studying transitions of care. (Learn more here.)
